All permanent employees (SHRA and EHRA, faculty and staff) must complete the online exit survey immediately AFTER you have informed your direct supervisor, either verbally or in writing of your intent to leave your position. You may write an email or letter to your supervisor IN ADDITION to this form, however, completion of this form is required. If this form is not received in a timely manner, it may impact the payout of applicable leave balances, or impact your benefits. Additionally, by answering the exit survey questions contained in the form, your feedback will help us in our goal of continuous improvement. It is very important to work with your supervisor in completing a property clearance form, and return all NCAT property in a timely manner.
